File Number in Text Message

| We are navigating the Mobile Intake forms rather well over the past several months. However, we have a need to send the clients their file numbers ahead of time. Is there a way for us to do this through merge fields? This would be very helpful. Thanks. |

| Glad Mobile Intake is working well for you. However, I'm not sure i understand your question. Why do clients need their file numbers? This number identifies an individual in your system and is considered PHI. You shouldn't sent or share PHI over unencrypted communication like SMS Text. |

Thank you for bringing this to our attention. We thought that because it was a generated number not associated with an identifier that it was acceptable.
In order to have complete records, we really need the ability to add custom questions and forms to the mobile intake process. Or we need the following questions part of the intake process/added to the client's case:
*Fillable Record Release documentation for clients to assign people whom they want to have access to their files
*children of the client - first and last name, date of birth, and current living situation
*Abuse Screening Questions: Are you a victim of abuse? *In the past year, has anyone ever threatened you or made you feel afraid? Has anyone ever hit, slapped, kicked, choked, or physically hurt you? Has anyone forced you to do something sexually that made you feel uncomfortable?
*Sexual Health History: In the past 90 days, have you: *Had sex with someone you did no know
Had sex with someone you met online or through a phone app, Accepted money or drugs for sex?,
Paid money or drugs for sex?, Had sex with a prostitute? (Male or Female), Injected any drugs into your body?, Been told that one of your sex partners has an STI/STD, Had sex with someone you did no know, Had sex with someone you met online or through a phone app
*Have you had any HCG injections or taken any HCG drops for weight loss?
*Are you able to contact your sexual partners to inform them if you STI/STD test returns positive? *
*How many sexual partners have you had? *Sex of partners *
*Are you experiencing any of the following symptoms? (Check all that apply) * |
