Tracking Client Exit Evaluations

Care Net is asking us to track our Client Exit Evaluations. Is this something that you can add somewhere in the client file? It looks like they just want an over all experience not specific details about the their visit.

We currently keep a file where we copy their evaluations so it is not too difficult to go back and count them. After the client has filled them out we put client file #, volunteer name and date on them before making the copy. The original stays in their file and a copy is placed in the exit evaluations folder. Being a small center it is not too difficult to track these manually, but I imagine larger centers may have more difficulty.

Hi Angie,

It's great to hear from you! GREAT news, you can track this today in CenterPiece. You have the ability to keep the exit response anonymous OR assign to a client file. Please follow the steps below to utilize this. If you have any other questions just let us know. Have a great day!

1. Login to CenterPiece
2. Click on Client Management
3. Along the Left hand side click "Exit Surveys"
4. Along the left hand side click "Add New Survey"
a. Specify staff/volunteer who saw the client
b. enter date, location, and rating
c. Tie to a client file OR leave blank to keep anonymous
d. Click "save and close"

I had forgot that you released a new update with that option! Thanks! It is very easy to use. I love that it will put it in my Care Net Report so I don't have to count by hand.

I have a few recommendations for making it even better --
- Allow a search for client by file number
- Show Survey Form in client file - on activity page or somewhere
- Have a place to enter it from the client file

As always, you all do a great job of making it easier to do our job! I love being able to do so many different types of reports and statistics and that you work so closely with Care Net to make it easy for us to complete our yearly statistics!

This was the only thread that I saw on exit evaluations, so I thought I would just stay with this tread. Thought it was funny that it is my thread from 5 years ago!! LOL

This seems very minor, but thought I would ask. When I pull up the exit evaluations it shows me the oldest entries first. I know that I can click on date to sort them by date or I could just go to the last page to see the most current. But would it be possible to make it automatically set to show the most current first? This would just make it easier and a bit quicker when we are checking to make sure we have entered them all in.

Again, this is minor, so it is not a problem if there is not an easy solution to this.
