Case Form Zip Code

Under the Case Form Question #17 asks for zip code. How is this being used? For statistics? We have putting one zip code for all the zip codes from that one county in this spot so we can run statistics for that county, but I just can't seem to differentiate this question from the client's actual address zip code! When I run statistics it seems to run the stats based on her actual address zip code not by the data code I put in Question #17.


Any my next question is...Is there any way to tell a time stamp or time line of who has charted in the chart and on what time and date. At my hospital job for electronic charting, they "time Stamp" it so they can see all activity and the user for each record. It would be nice to see this so I can tell who is doing what.

Hi joneski,

Yes, the zip code on the Case Form is used for statistics. What this allows you to do is still have accurate stats for past years even when the client changes zip codes and you change the contact information zip code. So regardless of how transient our clients are, we can always have accurate stats based on zip codes from prior cases. I hope this makes sense.

On some of our forms you will see a line at the very bottom of the page indicating who updated the form most recently, and the date/time of that update. For example, if you were looking at the Case Form at the very bottom (underneath the Save, Save and Close, and Close buttons), you'd see "Date Last Updated" and "Last Updated By" with a date/time and name of the person.

Ok, if I go to Statistics and run Appointments for Center and choose zip code the stats will only be ran using Question 17 data?

I have seen this time stamp, is there any other time stamp? If I had testify who did what to the chart, would I be able to see all activity?

Regarding the time stamp, it applies to the file information itself (so if you edited where all the contact information is), the case form, the pregnancy test from, the sonogram form, the note form, and the STD form.
