Assessment of Client's Pregnancy Intentions

On the Staff office use only Pregnancy Test Form, why do the options vary for Question #4
under Pregnancy test and Question #5 under the Sonogram. Seems like the options should be the same. Thank you for your help. |

You are right. This is an error on the Office Use Only Form. The options for questions #3 and #4 of the PT section should match questions #4 and #5 on the Sono section. For the time being (before we can get this corrected in CenterPiece), you will need to export the Office Form to Word, and add the additional options to numbers 4 and 5 of the Sonogram section on the Staff Office Use Only Form.
Note: Everything is correct in the actual data entry of the client's file. All options are present on the Sonogram Form and the PT Form.
