I realize that the new form is for places we refer our clients to, but what about the information we used to be able to get (how the client was referred to us)? Thanks!

Take a look at question: #1 - "How did you hear about us?" on the Intitial Visit Form. You can also customize the list of options. Hope that helps.

That is where I put that information in, but when I ran the reports for August stat's, it would not pull that information for me. It was trying to go to the new form and since there is no information there as of yet, it would not let me (although it would not have been correct if it had).

You mean the report did not show any data? or Your forms did not have any items selected? I'm not sure i understand based on your description above. To get these stats, run the Initial Visit stats are select the "HowHeardAboutUs" measure. hope that helps.

Where is the Initial Visit stats? I'm not seeing this option.
We are trying to get a report on how clients hear about us. Please advise.
