We need to know how to put information about a client into the system if they come in already knowing they are pregnant. They do not need us to do a test, but want to get involved in our programs. The pregnancy test form we have, only covers test done, no place for a due date if no test is done. |

Hi kboyer,
For these clients, you should still create a pregnancy test. This will help you track the client throughout her pregnancy. On the pregnancy test form, under office use only question "#2 Test Result", check the box that says the client provided a proof of pregnancy (pop). The statistics break out totals for positive pregnancies and positivie (pop) pregnancies. So you can tell which clients did not have a pregnancy test at your center.

Hi kboyer,
In this case you will still fill out the pregnancy test form, but in the Office Use Only section at the bottom you will flag that the test was a "Proof of Pregnancy". (This is a check box right under the test result field.) The “Proof of Pregnancy” flag means that you did not perform the test at your center, but the client came in already pregnant.
When you run reports, the report will categorize that test as a Proof of Pregnancy and not as a test done at the center. Therefore you will be able to track all the information around the pregnancy (due date, pregnancy intentions, etc.), but won't erroneously report that you did the test.

Sometimes we have clients come in who want to get involved with our programs but they have already had their babies. How do we apply information concerning the baby when we do not have a pregnancy test done? I tried the pregnancy history, but it says to go back and do initial pregnancy test form. |

Hi kboyer,
You should add the baby to the pregnancy history section of the client file. the explanation for pregnancy history is a little wrong. we will be correcting/updating it soon.
during the meeting with a client, you should use the Pregnancy History Office form to initially record this information. You can then add the information on this form to the database at a later time.
The office forms are just the "paper" versions of the data entry forms.
